Hi Simon,
I've done this sort of thing in a very lo-tech way years ago when I  
was doing research on video conferencing. The lo-tech way is simply  
to video the whole thing. You can usually get the video to an angle  
which allows one to see both people- the person and the screen.  
People have done this quite successfully to analyse interaction  
around computer screens.

Or one can use two cameras recording the skype window open on screen  
and the person's face and (lo-tech) put it through a box gizmo that  
security people use to split the screen in 2 or 4 or (hi-tech) use  
something like Final Cut Studio to create a split screen or picture  
in picture effect. You could vary it and have one more prominent  
depending on what you want.

One thing to note: if you are recording computer screens it's best to  
have a fairly good camera which - look out for one with specialist  
settings which allow the camera to synchronise its shutter speed with  
that of the computer refresh rate. otherwise you can get that wierd  
thing where the screen seems divided in half and half of it is darkish.
