Mark,

If the DVD is one you've mastered yourself you should be able to copy the
file structure and create a NTSC ISO image for burning using something like
DeVeDe<http://blogcritics.org/scitech/article/making-dvds-with-devede-in-linux/>

If the software you've used to master the DVD uses
css<http://en.wikipedia.org/wiki/Content_Scramble_System>protection
you'll need to rip using something like
HandBrake <http://handbrake.fr/> to rip it for you, then do the DeVeDe step
to create an NTSC image.
